Eligibility
- Entry must include evidence of completion or enrolment in a training program that is NZQA Level 3 or higher
- Not all questions need to be completed but the more information available the better for judging purposes
- Applications are open to organisations who are members of the Northland Branch of CCNZ.

Describe and summarise this person and why they been nominated.
This is what will be read out during the Awards Night presentation.
Attach no more than 5 photos you believe best represent the nominee to this question for considered for inclusion in the presentation shown on screen during the Awards Night (final photo selection is at the discretion of the committee).
2. Achievements
What are the Nominees most important achievements, highlights, or successes to date?
These can be completed by the Nominator and/or the Nominee

3. Performance Excellence
Describe an occasion (or occasions) where the Nominee has stood out as an excellent performer.
